This Companies House form is used to change the particulars of a director or the company secretary. This will include change of name, address, title, nationality or any other detail that was put on form 288a when the person was originally appointed.
You will need the following details:
- Company name and number
- Date the change(s) took place
- Title and name
- Date of birth (Directors only)
- New name
- New address
- Other changes
This form will need to be signed by any of the current directors, company
secretary, administrator, administrative receiver , receiver manager or receiver.

Completing and printing guidance
- Form must be printed on A4 paper 80-100 gsm2 white paper in a matt finish.
- Must be filled out in black bold capitals, either written or typed. Must be clear and legible.
- Do not use a dot matrix printer as this finish is not of sufficient quality for the Companies House scanner.
- Do not send photocopies.
Section 706 of the Companies Act 1985 allows Companies House to reject
documents that cannot be captured electronically, giving a notice saying why
they are unacceptable.
