Companies House Forms

Here are some of the most commonly-used Companies House forms. These forms are used to notify changes in the particulars of a UK company to Companies House, where the records of all UK companies are held. Read the instructions and notes very carefully before submitting any forms to Companies House.

288b - Terminate a director or company secretary appointment

288c - Change details of director or company secretary

287 - change registered office

88(2) - Return of allotment of  company shares

88(3) - Particulars of a contract relating to shares allotted as fully or partly paid up otherwise than in cash

122 - Notice of consolidation, division, sub-division, redemption or cancellation of shares, or conversion or re-conversion of stock into shares

123 - Notice of increase in nominal capital of company

128(1) -Statement of rights attached to allotted shares

128(3) - Statement of particulars of variation of rights attached to shares

128(4) - Notice of assignment of name or new name to any class of shares

155(6)a - Declaration in relation to assistance for the acquisition of shares

155(6)b - Declaration by the directors of a holding company in relation to assistance for the acquisition of shares

169 - Return by a company purchasing its own shares
